SERVICE COORDINATION
- Networking and outreach
- Facilitate Individual Support Plan (ISP) meetings
- Assist in the referral process to community service agencies for supports
- Provide support to individuals and families in a variety of planning meetings
- Assist in finding residential placement and employment services
- Make referrals to support services brokerages
Children from birth to age five who have a substantial developmental delay also qualify for services. These specialized services support children and their families to lessen the probability of a lifelong developmental disability, or reduce the effects of such a disability.
