Community Health Assessment

The Linn County Community Health Assessment (CHA) gives a comprehensive overview of the state of health within Linn County. The data used to assess the county's health includes collected health statistics, key informant interviews, and a quality of life survey. This data is also used as evidence for establishing priority areas in the Community Health Improvement Plan.

Community Health Improvement Plan

The Linn County Community Health Improvement Plan (CHIP) identifies key health issues within the county, and builds a work plan of goals and strategies to address these issues. The current CHIP's key health issues are Chronic Disease, Access to Care, Tobacco Use, and Substance Abuse.
