Maternal Child Health Programs

CaCoon Community Based Care Coordination

CaCoon is program for families with children who have special health care needs. CaCoon recognizes the importance of the family. Because you and other family members have a central role in the care of your child, services and resource assistance is planned with you and your pediatrician for your child.

How can CaCoon help?

A nurse care coordinator in your community with specialized training is available with resources and information to assist you in the coordination of services for your child.

Your nurse care coordinator can:

  • Screen children with a special health care needs and monitor their progress.
  • Identify developmental problems early
  • Provide medical information about your child's condition
  • Find and advocate for important services close to your home.
  • Teach special techniques for caring for your child.
  • Give support in times of stress and crisis.

CaCoon Care Coordination's goal is to help you and your family learn new skills to become as independent as possible in caring for your child.
