Maternity Case Management

A program for pregnant women and their newborn baby. A Public Health Nurse with special training works with Mom's and babies to promote the health and well-being of pregnant women and their newborn.


Services offered by this program for pregnant women and their babies are:

  • Home visits provide you with support and one on one time with a nurse.
  • Your nurse assists you and your family with your need for a referral to community services, classes and resources.
  • We help you find community support services for new mothers.
  • Your nurse will provide you with information about the care of your new baby.

What does this program cost?

Our program is voluntary and free to Linn County families.
