Help homeless or near homeless individuals/families overcome or prevent the conditions of homelessness by facilitating access to comprehensive community-based services leading to stabilized lives.

Referral Process

Any person or agency working with the homeless or near homeless individual may act as a sponsor.

A homeless or near homeless individual may also self refer and not require a sponsor.

Staffing Forms will be screened and staffed on a first-come/first-served basis. Applicant and sponsor will be notified of a staffing date and time.

The applicant and sponsor are expected to appear at the staffing unless other arrangements are made in advance.

The staffing will be done by multiple service providers. During the staffing an individual action plan will be developed. An advocate will be identified to assist the participant.

The progress of the participant will be reevaluated by AST after 60 days and for up to the first year.

At the end of the process, our goal is that each participant will have obtained and maintained permanent housing.

Staffing Form and Authorization for Release and Exchange of Information

Please download and complete all three of the forms at the bottom of the page:

  • The Adult Services Team Staffing Form must be signed by the sponsor.
  • The Authorization for Release and Exchange of Information must be signed by the participant.
  • The Expectations for Participating with AST must be signed by the participant.
